El rodaje del video musical del sencillo fue realizado los días 24 y 25 de septiembre de 2008 en el restaurante Takami Sushi & Robata y en Elevate Lounge en Los Ángeles,15 y contó con la participación del modelo estadounidense Brandon Stoughton, quien interpretó al personaje principal del mismo. Éste también incorporó un cameo de uno de los personajes del video musical de "Toxic". La línea de historia del video musical de "Womanizer" muestra a tres versiones absolutamente distintas de Britney Spears, interpretando tres oficios diferentes —secretaria, camarera y chofer de limusina—, mientras provoca sugestivamente la infidelidad de un hombre que responde a sus coqueteos y que demuestra no ser fiel a su pareja, la cual es interpretada por una versión dueña de casa de la cantante. De manera paralela a las escenas de su línea de historia, el video musical de "Womanizer" también incorpora escenas de la cantante completamente desnuda en un sauna. Su estreno fue realizado en el programa 20/20 de la cadena ABC el 10 de octubre de 2008.
